For students to be eligible to participate in a Fall Sport, an updated physical (Medical Form) and a completed middle school athletic form (Athletic Form) must be submitted. If you have questions regarding the medical forms, please contact School Nurse, Mrs. Saavedra, at (nsaavedra@wlps.org).

CONCUSSION TESTING
DATES: Tuesday 9/8
TIME: 2:30-3:10pm
LOCATON: Library
PLEASE NOTE: Students must arrange to walk or get a ride home at 3:10.
**ONLY for 6th graders who have not had a concussion test or any new student who did not have one last year**
